Junst 38b45d2e54 Fix BFloat16 dtype mismatch in concept map creation
- Ensure dtype consistency between embedding and attention tensors
- Convert embedding to attention_dtype before processing
- Fix projection layer dtype to match attention tensor
- Ensure interpolation tensors use consistent dtype
- Add proper dtype handling in fallback embedding creation
- Resolves 'expected scalar type Float but found BFloat16' error

Hook system working successfully with Linear layer selected (score: 9437234)
Attention output shape: torch.Size([1, 1024, 9216]) - ready for processing!
