Mitch b34cb393b9 docs: rewrite README + add CHANGELOG and example workflow for v2 ...

* README documents new modes, format/index_str output, reset button,
  and adds a dev section covering uv, ruff, pyright, pytest.
* CHANGELOG follows Keep a Changelog with entries for v2.0.0, v1.1.1,
  v1.1.0, v1.0.0.
* workflows/hold_counter_basic.json: minimal example wiring HoldCounter
  to LoadImage + PreviewAny.
