Fix concept embedding extraction and enhance attention capture

🔧 **Critical Bug Fixes**:

1. **Fixed Concept Embedding Extraction**:
   - Fixed 'NoneType' object is not callable error
   - Added proper null checks for text_encoder and tokenizer
   - Added fallback embedding creation with proper dtype
   - Enhanced error handling for embedding extraction

2. **Enhanced Attention Capture**:
   - Added apply_model execution to force hook triggering
   - Improved direct attention module execution
   - Better fallback mechanisms for attention capture
   - Enhanced logging for debugging

3. **Robust Error Handling**:
   - Better null checks and fallbacks
   - More comprehensive error logging
   - Graceful degradation when components are missing

This should resolve the concept embedding extraction error
and improve attention output capture significantly.
