Align with original ConceptAttention implementation and fix expected errors

Major improvements based on helblazer811/ConceptAttention:

1. **Concept Map Generation**:
   - Use attention-weighted concept relevance instead of simple cosine similarity
   - Implement proper projection from concept embedding to attention space
   - Follow original paper's approach for concept-attention interaction

2. **Attention Output Selection**:
   - Intelligent selection of best attention output based on layer type and size
   - Prefer attention layers over norm layers for better concept extraction
   - Score-based selection for optimal spatial information

3. **Robust Interpolation**:
   - Handle multiple tensor formats (2D, 3D) for interpolation
   - Add fallback resize method using scale_factor
   - Comprehensive error handling for dimension mismatches

4. **Improved Normalization**:
   - Use min-max normalization instead of softmax for better visualization
   - More stable concept map generation
   - Better contrast and interpretability

5. **Enhanced Embedding Processing**:
   - Learnable projection layers instead of simple padding/truncation
   - Proper normalization for projection stability
   - More aligned with original ConceptAttention approach

6. **Error Prevention**:
   - Comprehensive tensor dimension handling
   - Fallback mechanisms for interpolation failures
   - Better error messages and logging

This implementation now closely follows the original ConceptAttention paper
while being adapted for ComfyUI's ModelPatcher system.
