Implement real ConceptAttention extraction from Diffusion Transformers

MAJOR IMPLEMENTATION CHANGES:

1. Real Attention Extraction:
   - Extract actual attention outputs from DiT model using forward hooks
   - Capture attention weights from attention/attn layers
   - Process attention outputs to create spatial attention maps

2. CLIP-based Concept Matching:
   - Extract CLIP embeddings for individual concepts
   - Compute cosine similarity between attention and concept embeddings
   - Generate concept-specific saliency maps based on attention-concept similarity

3. Enhanced ConceptAttentionProcessor:
   - Real model forward pass with proper DiT input formatting
   - Attention hook registration and cleanup
   - Fallback to mock data if real extraction fails

4. Improved Segmentation Logic:
   - Use real concept maps when available
   - Apply attention thresholding for segmentation
   - Fallback to sophisticated mock patterns for missing concepts

5. Better Error Handling:
   - Graceful fallback to mock data on errors
   - Comprehensive logging for debugging
   - Multiple model call pattern support

This implementation now attempts to extract real attention from Diffusion Transformers
and match it with CLIP concept embeddings, providing actual ConceptAttention results
while maintaining fallback capabilities for robustness.
