Fix: same model reference in different nodes not both detected
...
When two different nodes referenced the same filename but with different
node_ids/widgets, the deduplication logic skipped the unsolved node
because resolved_models used only (category, basename) as key.
Added exact_resolved per (name_lower, node_id, widget) to only skip
adaptable/missing items when the exact same node+widget is already
installed.
