Add a 2026.11.04 notice to README explaining that many node inputs and
outputs were renamed or reordered and need reconnecting, and update the
ja/en/zh node docs and changelog for LIST/ARRAY, x / y_items, the new
D2 List Converter and the deprecated list nodes.
